Aged Care Coordinator
Salary: $75,000 - $80,000 subject to experience
Position summary:
The Aged Care Coordinator is responsible for coordinating and overseeing the delivery of in‑home aged care services under the Support at Home Program. The role ensures that older people receive high‑quality, reablement‑focused supports that promote independence, wellbeing, and safety. The Care Coordinator works closely with clients, carers, support workers, and multidisciplinary teams to deliver responsive, goal‑oriented services aligned with program requirements and aged care quality standards.

Role Description Support Coordinator
Client Intake, Assessment & Onboarding

Facilitate intake processes for older people seeking Support at Home services.

Gather relevant personal, functional, and social information to support assessment and planning.

Coordinate or contribute to comprehensive assessments that identify needs, risks, and reablement opportunities.

Ensure all onboarding documentation meets regulatory and program requirements.
Reablement‑Focused Care Planning & Coordination
-
Coordinate delivery of in‑home supports including personal care, domestic assistance, social connection, assistive technology, and home modifications.

Adjust service plans as client needs evolve, ensuring continuity and responsiveness.

Collaborate with multidisciplinary teams to integrate clinical and non‑clinical supports.
Monitoring, Review & Quality Assurance
-
Conduct scheduled reviews to monitor progress toward goals and evaluate service effectiveness.

Identify changes in client condition and escalate concerns to clinical or allied health professionals.

Maintain accurate, timely case notes and service records in line with Support at Home reporting requirements.

Audit service delivery against care plans, budgets, program guidelines, and organisational policies.

Implement continuous improvement actions based on feedback, incidents, or audit findings.
Workforce Coordination & Support

Provide clear direction to support workers regarding client goals, reablement strategies, and risk considerations.

Ensure staff have required competencies, clearances, and training to deliver Support at Home services.

Assist Staff Scheduler with coordination of rostering, backfilling, and urgent service changes while maintaining continuity of care.

Facilitate communication between field staff, allied health, and the broader care team.
Risk Management & Safety Oversight

Conduct or coordinate home safety assessments and develop risk mitigation strategies.

Identify hazards such as mobility risks, environmental hazards, or behavioural concerns.

Respond to incidents, complete incident reports, and coordinate follow‑up actions.

Ensure compliance with safeguarding, mandatory reporting, and duty‑of‑care obligations.
Client, Carer & Family Engagement
-
Maintain regular communication with clients and informal carers to ensure satisfaction and address concerns.

Support clients to understand and navigate the Support at Home Program.
-
Assist with transitions such as hospital discharge, increased care needs, or palliative care.
-
Advocate for client needs within the organisation and with external providers.
Administrative & Compliance Responsibilities

Maintain accurate client records, service agreements, and consent documentation.
-
Ensure compliance with aged care quality standards, privacy legislation, and organisational policies.

Prepare reports for internal management, quality audits, and program compliance.
-
Track service utilisation, budgets, and funding allocations under the Support at Home model.
-
Use client management systems to record data, schedule services, and generate required reports.
Community & Provider Liaison
-
Build and maintain relationships with external providers, hospitals, allied health, and community organisations.

Coordinate referrals for reablement services, assistive technology, home modifications, or specialist supports.

Participate in case conferences, multidisciplinary meetings, and community engagement activities.
Qualifications and experience required
Skills and Competencies
-
Strong organisational and time‑management skills, with the ability to manage multiple clients and prioritise urgent needs.

Excellent written and verbal communication skills, including care planning, documentation, and progress reporting.

Ability to conduct holistic assessments, identify risks, and develop practical support plans that promote independence and wellbeing at home.

High level of empathy, patience, and professionalism when working with older adults and their families.
-
Competence with digital tools, case management systems, and remote communication platforms.
Knowledge Requirements
-
Understanding of aged care frameworks, including Support at Home (SAH), Commonwealth Home Support Programme (CHSP), and relevant legislation or quality standards.

Familiarity with safeguarding principles, elder abuse prevention, and mandatory reporting obligations.
-
Knowledge of community resources, clinical supports, and in‑home service options available to older adults.
Professional Experience
-
Minimum 3 years’ experience in a similar Care Coordinator role, preferably within aged care, home care packages, community care, or related support services.
-
Demonstrated capability in coordinating in‑home supports for older adults, including navigating changing care needs and ensuring continuity of services.
-
Proven experience collaborating with clients, families, carers, allied health professionals, and service providers to deliver safe, person‑centred care.
Work Conditions
-
The role includes a 3‑month probation period to ensure suitability and performance alignment.

Following onboarding and training, the position will transition to remote work, requiring reliable internet access and a suitable home workspace.

Occasional in‑person visits may be required depending on client needs and organisational requirements.
Other Requirements

Current Police Check and Working With Children Check (as applicable).

Valid driver’s licence and access to a reliable vehicle for any required home visits.

Commitment to ongoing professional development in aged care and person‑centred practice.

Desirable - Ability to speak Arabic or second language
